In 2016–2018, there were 77 accidents that occurred in the commercial ship and general engineering divisions. This research aims to identify the basic causes of accidents in order to prevent similar accidents from happening again. prevent similar accidents from happening again. This research will use the RCA method with a five-why analysis and a fishbone diagram. The results of the five-whys analysis show the facts that cause accidents in a structured manner from both victims and other witnesses. The results of the fishbone diagram show that the factors that cause accidents from each event include human factors, machines, materials, environment, methods, and measurements, making it easier to determine the basic causal factors. Of the 10 accidents analyzed, 4 cases in the general engineering division and 6 cases in the commercial ship division were obtained.
